Required for Tryouts:

OHSAA Physical - IMPORTANT, all information must be completed by the appropriate people. Please make sure the doctor uses this form and that you and your student-athlete sign all areas.

Emergency Medical Form (Even if this your 4th time filling out this year we need another. My advice in the future is to make copies at the beginning of the year)
*All Emergency Medical Forms can be printed from your child's infosnap account.

Required for Season (once student has made team):

Parent- Athlete Agreement Form

Concussion Form

Pay to Participate (If this is your first sport of the year)

Acknowledgement of Insurance Plan (If needed)








Other:
Travel Release Form (This form is only needed if you need to take your student-athlete home after an away game. Talk to your coach ahead of time if you plan on doing this.)
